Proxy Program

Collins Library's Proxy Program allows UPS Faculty to designate one or two persons (such as the department's administrative assistant or student assistant)  to act as representative when checking out library materials, making copies from microforms, or requesting interlibrary loans for faculty research. This representative must be affiliated with the university while acting as proxy. 

Faculty members who wish to designate a proxy should complete a proxy request form for each person.  Please print it out and mail it to Cassandra Palmore at the library. You may also bring it to the Circulation Desk. 

By designating another person to act as proxy, faculty are authorizing that person to check materials out in their name. Faculty become responsible for the replacement costs, service charges, and any overdue fines that accrue for materials checked out by proxy. If the proxy leaves UPS or is no longer working for the faculty member,  please notify the library as soon as possible. 

Please note that the designated proxy must present a valid picture ID for identification at the Circulation Desk.
